A) Have the camper's medical provider complete the CFR Medical and Dietary Forms.
NOTE! These forms require the signature of the camper's licensed medical professional. An appointment should be made as soon as possible.
B) The Camper Behavior Agreement form must be signed by both the camper and the parent/guardian. The Camper Behavior Agreement establishes the understanding to abide by the items outlined in the Camp Floyd Rogers Handbook found below in section four.

The completed forms must be uploaded online or mailed to the Camp Floyd Rogers PO Box by June 1st. Please do not email the camper forms. The camper forms must be completed in its entirety which includes the medical provider's signature. Only camper forms completed in its entirety are marked as received. Camp WAIT LIST
We try to include all children that want to come to Camp Floyd Rogers but are limited by space. However, we do have cancellations and regulary use the waiting list. Once on the waiting list, a confirmation email will be sent to the parent/guardian email addresseses. Those email addresses will be used for all camp communication.
