Camp Floyd Rogers Application Process


Application's should NOT be sent in before February 1st, 2012


1) Fill out the CFR Application Form and mail with the registration fee to:

Please do not proceed until the camper is accepted!

2) Once accepted the following need to be completed and mailed to the CFR Office at the above address:

              a) CFR General Rules For Camper Conduct Form

              b) CFR Camper Information Form

              c) CFR Interesting Facts about YOU!

3) The following need to be completed and brought with the camper during check-in:

              a) CFR Health History And Examination Form
                      (NOTE! This form requires the signature of a licensed medical professional.
                      An appointment should be made with the child's doctor as soon as possible)

              b) CFR Walk For Wellness Form
                      (NOTE! The funds raised will help to underwrite research for a cure and
                      new treatments for diabetes. Start early and raise as much as possible.)

              c) CFR Insulin Pumper Form
                      (NOTE! Only required if the camper has an insulin pump.)

              d) CFR Camper Meal Plan Information Form
